Job Description:
We are seeking a dedicated and compassionate TFC (Therapeutic Foster Care) Case Manager to join our team in the Conyers Office. The TFC Case Manager will play a crucial role in providing support and guidance to foster families and the children placed in their care. This position requires a deep understanding of the needs of children in therapeutic foster care, as well as the ability to collaborate effectively with various stakeholders, including families, social workers, and other service providers.
Key Responsibilities:
- Case Management: Manage a caseload of children in therapeutic foster care, ensuring that each child’s needs are met and that they receive appropriate services and support.
- Assessment and Planning: Conduct comprehensive assessments of children and families to develop individualized service plans that address emotional, behavioral, and developmental needs.
- Foster Family Support: Provide ongoing support and training to foster families to ensure they are equipped to meet the needs of the children in their care.
- Collaboration: Work closely with social workers, therapists, schools, and other community resources to coordinate services and ensure effective communication regarding each child’s progress.
- Monitoring and Evaluation: Regularly monitor children’s progress and the effectiveness of implemented strategies, making adjustments as necessary to improve outcomes.
- Documentation: Maintain accurate and up-to-date case files, including notes on meetings, progress reports, and any relevant correspondence.
- Crisis Intervention: Be prepared to provide immediate support and intervention in crisis situations, ensuring the safety and well-being of the child.
- Advocacy: Act as an advocate for children and families, ensuring their voices are heard in all aspects of care and services.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field (Master’s degree preferred).
- Minimum of 2 years of experience working in child welfare, foster care, or a related field.
- Strong understanding of therapeutic foster care principles and practices.
- Excellent communication and interpersonal skills, with the ability to build rapport with children and families.
- Ability to work collaboratively as part of a multi-disciplinary team.
- Strong organizational skills and attention to detail.
- Proficient in Microsoft Office Suite and case management software.
- Valid driver’s license and reliable transportation.
Preferred Qualifications:
- Previous experience as a case manager or in a clinical role.
- Knowledge of local resources and services for children and families.
- CPR/First Aid certification.
